When a youth is involved with the Ministry of Children and Family Development (MCFD) it can feel overwhelming. The collaborative planning coordinator supports the youth to develop strong connections between people and agencies who are involved in their life.

Collaborative planning creates space for conversations between a youth, family, and the ministry. Our coordinator is a facilitator who keeps meetings focused, makes sure everyone feels safe and respected, and ensures every voice is heard.
Youth Transition Conferences focus on young people approaching adulthood and exiting care. These meetings identify who will still be there after the youth turns 19 and help with planning for housing, education, and future goals. In Campbell River, The John Howard Society of North Island facilitates these meetings when requested by MCFD or by families directly.

Get Started
STEP 01
MCFD referral
Referrals are received through the Ministry of Children and Family Development (MCFD). A social worker will submit a referral on behalf of the family.
STEP 02
We connect
Our team will follow up with the social worker and family to coordinate next steps and begin planning together.

The coordinator works for The John Howard Society of North Island. Their role is to make sure everyone’s voice is heard and the meeting stays on track.
The coordinator’s role is to facilitate meetings between you, your supports, and social workers as a group. They can refer you to other professionals who can help you come up with a plan and support you to work toward getting your children back.
- Family Case Planning Conference (FCPC) is organized by the social worker. The social worker invites family members and community workers involved with the family. It is usually 90 minutes long and creates a short-term plan to address safety concerns.
- Family Group Conference (FGC) brings together family, professionals, and others who care about the children to create a long-term plan.
- Youth Transition Conference (YTC) is a meeting that creates a support network around a youth as they approach adulthood and exit MCFD care. It identifies people who will be there after the youth’s nineteenth birthday and assists with planning for future goals.
